Healthy Hearts is a non-profit organization and a non-funded cardiac rehabilitation program. Fees go toward the payment of rent of our facility, staff wages, equipment maintenance, and purchase of supplies.
To make payment of the fees convenient the following options are available:
Phase One of the Program
Full Payment, One Cheque for 3 months: $150
Refund of fee for full months remaining if participants withdraw from program.
Monthly, 3 postdated cheques for 3 months: $50
Return of unused cheques for full months remaining if participants withdraw from program.
We have a cashless system at Healthy Hearts so cheques or automatic withdrawal are the preferred method of payment. Forms for Automatic Debit option are available at Healthy Hearts office. A voided cheque is required by the bank. Information is strictly confidential.
Phase Two of the Program
Semi-Annually
One Cheque for 6 months: $300 / Single OR $450 / Couple
Monthly
6 Postdated cheques for 6 months: $50 / Single OR $75 / Couple
Automatic Debt
Monthly withdrawl: $50 / Single OR $75 Couple
Refund of fee for full months remaining if participants withdraw from program or cancellation of automatic withdrawl with one months notice.
